 NHỮNG KẾT QUẢ MỚI CỦA LUẬN ÁN

1. Đề tài luận án đã xác định được tỷ lệ bệnh tăng huyết áp (THA) ở người cao tuổi (NCT) tại tỉnh Hưng Yên khá cao (28,2%). Công trình nghiên cứu được thực hiện tại một tỉnh vùng đồng bằng chưa triển khai chương trình quản lý điều trị bệnh THA tại cộng đồng. 

2. Mô hình kiểm soát THA đã huy động được nguồn nhân lực tại địa phương, thu hút sự tham gia của cộng đồng, phối hợp chặt chẽ giữa trạm y tế, nhân viên y tế thôn bản và hội NCT, các tổ chức đoàn thể khác trong xã hội để hoạt động có hiệu quả. Biện pháp quản lý điều trị đã giúp cho NCT tiếp cận dễ dàng với dịch vụ y tế. Chăm sóc y tế tại nhà, theo dõi huyết áp NCT tại hộ gia đình để phát hiện bệnh THA ở NCT, phân loại bệnh THA của NCT để điều trị tại nhà, khi nặng thì chuyển lên y tế tuyến trên (bệnh viện). Mở các lớp tập huấn nâng cao năng lực thực hiện mô hình kiểm soát bệnh THA: giúp cho cán bộ y tế cơ sở, lãnh đạo cộng đồng và cán bộ hội NCT về kỹ năng quản lý, điều trị huyết áp và truyền thông phòng chống THA cho NCT. Đây là cơ sở quan trọng để giúp cho việc thực hiện các giải pháp quản lý, chăm sóc sức khỏe cộng đồng có hiệu quả và bền vững.

3. Mô hình đã can thiệp vào kiến thức, thái độ, thực hành về phòng biến chứng bệnh THA, về điều trị bệnh THA và về dự phòng THA cho NCT. Kết quả đã làm thay đổi được những hiểu biết sai lầm về bệnh THA, thay đổi được những hành vi bất lợi cho bệnh THA nhờ đó đã làm thay đổi được mức độ THA, giảm được tỷ lệ bệnh THA và các tai biến do bệnh THA gây ra.

* Các ứng dụng, khả năng ứng dụng trong thực tiễn:

Cách thức tổ chức, mạng lưới kiểm soát THA ở cơ sở, tài liệu truyền thông về dự phòng THA cho NCT đã ứng dụng thành công tại 2 xã phường can thiệp sẽ tiếp tục được duy trì và nhân rộng ra các xã phường khác của tỉnh Hưng Yên.

* Những vấn đề bỏ ngỏ cần tiếp tục nghiên cứu:

Nghiên cứu mô hình kiểm soát bệnh THA ở phạm vi rộng hơn không chỉ riêng cho đối tượng người cao tuổi mà còn cho đối tượng người trưởng thành. Đồng thời để đánh giá một cách toàn diện, cần nghiên cứu thêm những yếu tố ảnh hưởng tới quá trình triển khai mô hình như: chất lượng nhân lực, trang thiết bị, thuốc điều trị và các cơ sở vật chất phục vụ cho hoạt động của mô hình.

 THE NEW SCIENTIFIC FINDINGS

1. The thesis identifying the prevalence rate of hypertension is rather high in elderly in Hung Yen province (28.2%). The study was carried out in a delta province where the programe of hypertention management was not implemented in the community. 

2. The hypertension control model has mobilized a local human resource, attracted the participation of community, coordinated closely between the Commune Health Center, village health staff and Association of Older People, other mass organizations in the society to operate effectively. Treatment and management measures have helped older people easy access to health services. Home health care, blood pressure monitoring in elderly households to detect hypertension in elderly patients, classification of hypertensive disease to treat in the elderly households, and when their disease become serious, they will be referred to the higher health level (hospital). Opening training courses on building capacity of ​​hypertensive disease control model: it will help health workers in health grassroots, community leaders and the officials in Association of Older People improve the management skills, blood pressure treatment and  communication on control of hypertension for older people. This is an important basis to help the implementation of management solutions, community health care  effectively and sustainably.

3. The model has intervened in knowledge, attitude, practice on prevention of complications of hypertensive disease, the treatment of hypertension and hypertension prevention for the elderly. The results have changed the wrong understanding of hypertensive diseases, changed unbenefited behaviors toward hypertension and owing to that, it has changed the grade of hypertension, reduced the incidence of hypertension and hypertension - induced complications.

 PRACTICAL APPLICABILITY AND THE NEEDS FOR FURTHER STUDIES

* Practical applicability:

The way to organize and to build networks of hypertension control in grassroots, to develop materials on prevention of hypertension for older people has successfully applied in two intervention communes/ wards and it will continue to be maintained and considered for a large - scale replication in other communes/wards in Hung Yen province and if having conditions.

* Opening issues for further study:

Study of hypertension control model should be implemented in lager scale not only for older people in particular, but also for adults in general. At the same time, to evaluate comprehensively, further studies on factors affecting on the model deployment process need to be done such as: quality of manpower, equipment, medication and facilities for activities of model.
